Ashtabula County Ohio Medical Center Incorrectly Billed Medicare for Leuprolide Acetate Implants (A-03-12-00010) http://go.usa.gov/79D

Ashtabula County Medical Center (the Medical Center) incorrectly billed Medicare for all of the 14 line items billed for leuprolide acetate implants, rather than leuprolide acetate injections. The Medical Center billed Medicare using an incorrect Healthcare Common Procedure Coding System code and incorrect units of service. As a result, the Medical Center was overpaid a total of $258,000.

Recovery Act Costs Claimed by Health Services, Inc., in Alabama Were Generally Allowable (A-04-11-03541) http://go.usa.gov/79W

Health Services, Inc.’s, claimed costs generally were allowable under the terms of the grants and applicable Federal regulations. The Health Resources and Services Administration awarded the grantee $14.3 million under the American Recovery and Reinvestment Act of 2009.

William Smith, Sr., Tri-County Child Development Council, Inc.’s, Financial Management Practices and Systems in Texas Did Not Always Meet Federal Requirements (A-06-11-00036) http://go.usa.gov/79Z

The William Smith, Sr., Tri-County Child Development Council, Inc. (Council), a nonprofit agency that operates Head Start and Early Head Start programs in several Texas counties, did not always meet Federal requirements in its financial management practices and systems. Specifically, the Council claimed $27,000 in unsupported consultant services costs; misclassified Recovery Act, nontravel, and administrative costs in its accounting system; did not have adequate physical controls and did not adequately segregate duties in its accounting department; did not adequately segregate its procurement duties; and claimed $32,000 in unallowable in-kind non-Federal share.
